1) I have the following configuration:
printer=true
dpi=360
width=85
height=110
printoutput=printer
multipage=false
docpath=.
timeout=0

[parallel]

parallel1=printer
parallel2=disabled
parallel3=disabled

It still goes out to a png file when getting printed.

2) Do I need to redirect the output to the usb printer if this *printer* setup goes out to the real lpt1 port when I want to print with an usb printer?

But Do you have a windows 3.1 graphics driver for the virtual printer?

Yes, i got it, for win31, I can use epson driver provided by MS
but the quality is barely acceptable
