'PHD69N03LT' - there is your faulty component. Replace it and that should take care of the 12v rail. I personally like to replace also the dual fast switching rectifier when used in this config, but it's a matter of choice.
Now to the 5v and 3.3v rail , do you still have the transformer ? Can you post a pic with it ? Maybe he's not that damaged .
